  A 32-year-old nulligravid woman presents to the office with gradual onset of body hair over the past 6 months. She states she has overwhelming fatigue with much less energy than she used to. Her menses, which used to be regular and predictable are now irregular and unpredictable. Her family history is negative for any similar complaints. Upon general examination her face appears round with increased upper body obesity especially around the neck and back. She has stretch marks on her abdomen, thighs, buttock, arms, and breasts. Pelvic examination reveals normal female external genitalia with a normal-sized clitoris. On bimanual examination you palpate a normal midline, mobile, nontender uterus with no adnexal masses.
