  A 39-year-old woman presents to the outpatient clinic with a 12-month history of increasing coarse, darkly pigmented facial and body hair. She has a history of extensive endometriosis, with pain so severe and disabling that she underwent a total abdominal hysterectomy and bilateral oophorectomy. She was placed on estrogen therapy, which has been successful in treating vasomotor symptoms and vaginal atrophy. However, her libido markedly decreased after the surgery, so a year and a half ago she was placed on combination estrogen and testosterone therapy. Her libido has normalized since the added androgen therapy. On physical examination you find increased generalized body hair. Pelvic examination reveals a normal-sized clitoris and a normal vagina and uterus without adnexal masses.
