92. 20 month old african american boy brought for fatigue and lethargy to office/ Fe deficiency
93. Acute Bacterial Prostatitis
94. ALL in a 5 year old/ 5 yr. old boy who came with weakness, disinterest in activity and lesion on leg.
95. Acute pericarditis – rx ( make sure to do echo, dont do unnecessary pericardiocentesis if there is mild to moderate pericarditis with out clinical or echocardiographic evidence of tamponade)
96. Osteoarthritis of the Knee ( if there is large joint effusion, always do arthrocentesis)
98. Congestive heart failure in a post-op patient ( make sure they are not giving too much IV fluids in post op setting, I/O monitoring, daily weights, lasix, 2d echo, r/o MI, EKG, CXR, BNP – Lasix, if flash pulm edema, give morphine)
99. Hypercalcemia/ renal mass ( likely RCC) – Elderly man presenting with fatigue
100) Complete Heart Block - Woman coming with Motor Vehicle Accident/ only minor injuries on the arm , Vitals reveal Heart rate 38. - EKG shows complete Heart block

  PULMONARY EMBOLISM ( remember what Archer says in CCS workshop - DLMTS - Dance Like The Movie Star
for DIAGNOSIS, LOCATION, MONITORING, TIMING, SEQUENCING - all these elements must be addressed in a CCS case to get the best score).

ER presentation with chest pain and shortness of breath. History of breast cancer. Vitals are stable.
OXY : pulse ox, stat ; oxygen, stat, continuous
cardiac monitor, stat, continuous
BP monitor, stat, continuous
aspirin sublingual
elevate head of bed
PE-- Heart, lung, ext

EKG, stat
ABG, stat
cxr, stat
cbc, stat
bmp, stat
Pt/Inr, stat
Ptt, stat
cardiac enzymes
Physical showed one extremity swollen

Heparin IV, stat, continuous ( do not wait until doppler comes since pre-test probability of VTE is high)
Blood pressure 120/80 so not an unstable PE. ( no need for tPa)

Venous doppler bilateral
Spiral CT stat
D-dimer, stat
Change Location : ADMIT TO WARD :

Bed rest, complete
pulse ox q2
vitals q4
Ptt q6
while on heparin, start coumadin
Pt/inr daily. Once INR > 2.0, d/c heparin


anticoagulation teaching
pt counseling
pt/inr 2 days
Factor V leiden, Factor II gene mutation, APLA panel.
On calender set up later tests - PT/INR in 2 days ( MONITORING)
