* Q. Postmenopausal bleeding dx approach

  I thought post menopausal bleeding is an indication for Endometrial biopsy.
But I'm confused when it is necessary to do pelvic US before the biopsy.

Is Pelvic US the initial test that needs to be done before Endometrial biopsy?

  it helps to evaluate for the thickness and any polyps/fibroids. so if the thickness level is over a certain level or if polys are present/fibroids--you can further evaluate with a biopsy as needed...if everything is normal, than a biopsy is not needed.

I'm more curious whether US evaluation is required as the initial diagnostic tool (for the purposes of CK exam).

UW question had US as an answer choice and the explanation sounded like as if US is REQUIRED initial diagnostic tool (which I don't agree).

In your opinion,
If choices given, which one comes first? US or biopsy?

  US first still--because statistically speaking, atrophy of vaginal mucosa is number 1 for postmeno. bleed; but of course, ruling out CA is important. also, you don't want to be invasive right away right? So US first and then biopsy.

I previously would of gone with biopsy first (before i started studying for CK), so no worries, I can see why you went that route first!
