  A 42-year-old man is brought to the emergency department because of a 2-day history of muscle spasms and jaw stiffness. He is sexually active with three partners and uses condoms inconsistently. He has a 5-year history of intravenous heroin use. He appears irritable. His temperature is 38.8°C (101.8°F), pulse is 120/min, and blood pressure is 152/96 mm Hg.
