how can we start the LOR??

letter of recommendation

Actually I have 3 LOR with the above mentioned three different statements.

do my LOR work.
anyone.....

Dear Program Director:

Although ERAS advised us to get a LOR addressed to PD but practically it does not matter.

IT DOES MATTER !!!
If your letter starts as "to whom it may concern" it is an immediate downpull to anyone reading the LOR. While other startings can be fine, I would advise you to avoid this opening at least.
