Dear everyone,
I am so confuse to manage a new onset AF. Could somebody tell me when should I initial cardioconvertion and how: Pharmacal or eletrical. And when should we start anticoagulate?

Atrial fibrillation
Fisrt look whether STABLE or UNSTABLE

UNSTABLE
Cardiversion

STABLE

Slow the HR using DIGOXIN/CCB/BETA BLOCKERS

Then Convert them CHEMICAL OR ELCTRICAL CARDIOVERSION

If less than 48 hrs no anticoagulation

If more than 48 hrs then either do a TEE to look for clot if no clot go ahead with Chemical /Electrical Cardioversion

Or give 1 month of Warfarin

Chemical Cardioversion is with drugs- Amiodarone,Ibutilide, Flecainide,Sotalol

After conversion -1 month of Warfarin

If all FAILS then life long WARFARIN
