1) ur answer would be: mr. Y , i advise u to be admitted in the hospital for further work up of ur problem n to observe ur response to therapy . outcome wil depend on ur body's reaponse to therapy n blood test results , we will try our best to help u.
( till my knowledge severe bleeding cases r not there as they r ER cases . might pt cm with a small bout of blood n in that case u can say this much bleeding can usually not lead to death bt us should be in the hospital for further work up n observation.)

2)yes , ideally we should ask CAGE ques. to all alcohol intake pt. . eventhugh it depends on time management n the case . as in cns case u will not get that much time n u feel other part of counselling is more imp u can skip it . (in case of arthritis u suspect gonococcal arthritis, ur counselling is must on sexual practise , at that time u can skip alcohol CAGE ques. if time doesnt allow u .)

3) no ,u will not write wat u havent done. also if u have look for JVD and u didnt explain it to the SP u should not write . as SP wil give a report of ur performance too.

4)v have to cvs exam in all three position if u feel the case history is in favor of cardiac cause /diagnosis .other wise only auscultation in any position is sufficient.

5)range of motion is passive . v have to do it , taking care that SP wil not be hurt .

other resposes r welcomed .

thank u .
