Here is finding from CMDT. CMDT has given ref from ATP III: Please correct me if my understanding is wrong:

"Very high risk"
favors the optional LDL cholesterol goal of < 70 mg/dL, or in patients with high triglycerides, non-high density lipoprotein (HDL) cholesterol < 100 mg/dL


If baseline LDL cholesterol is < 100 mg/dL, institution of an LDL-lowering drug is a therapeutic option on the basis of available clinical trial results. If a "high-risk person" has high triglycerides or low HDL cholesterol, combining a fibrate or nicotinic acid with an LDL-lowering drug can be considered.

Very high risk group:
-----------------------------

known coronary artery disease," OR" CAD equivalent (DM/PVD/symptomatic carotid artery disease(TIA/STROKE)/AAA/ 10yrs risk of CAD less than20 %)

AND

multiple risk factors (smoking/ family history... age./HTN/ (can be in more combination)

as example:
extablished CAD + DM
DM+ smoking
DM + F/O/ premature CAD
and so on.
