Self reported painless testicular mass in a young adult---> Do we do U/S or orchiectomy?

Please explain why either choice. Thanks!
usg, then radical orchiectomy

Scrotal ultrasonography is nearly 100 percent accurate in distinguishing between intratesticular and extratesticular pathology. All intratesticular masses are considered cancer until proved otherwise.
