TIBC in Fe overload/sideroblastic anemia?
FA says normal
Goljan says low
What do you say?
Sideroblastic anemia: Serum transferrin (and TIBC) normal to low, serum iron normal to high, saturation high
too much iron....so TIBC will be low.....
agree with the others: TIBC is an indirect measurement of transferrin. Transferrin is synthesized in the liver, but its synthesis is inhibited by Ferritin. In iron overload (or anemia of chronic disease), there is excess iron in the bone marrow so Ferritin is high, causing decreased transferrin. Therefore, TIBC is decreased.
thanks, that's exactly what I thought but needed to check
