47-year-old man has experienced progressive, symmetrical muscular weakness for several years. He has no myalgias or arthralgias. His mental status is unchanged. There is no history of traumatic injury. Within the past two months he has had increasing difficulty with swallowing and speaking. On physical examination, fasciculations of the tongue are noted.
