sports medicine - histomorph - ArchivalUser - 10-27-2007 A 20-year-old man comes to the health center because of ankle pain. Two days ago he sustained an inversion injury of his left ankle in a basketball game. He has been able to walk unassisted since the injury. Today he has pain and moderate swelling and discoloration over the lateral malleolus. Physical examination shows tenderness on palpation over the anterolateral corner of the ankle joint. He has had two similar injuries in the past. Which of the following is the most appropriate initial management? A) An ankle-strengthening exercise program B) Application of a long-leg cast for 3 weeks C) Application of a short-leg cast for 3 weeks D) Protected weight bearing E) Surgical repair of the ankle ligaments 0 - ArchivalUser - 10-27-2007 E) Surgical repair of the ankle ligaments 0 - ArchivalUser - 10-27-2007 E should be right 0 - ArchivalUser - 10-27-2007 this patient was weight bearing...now c/o pain after 48hrs...there is tenderness on the anterolateral corner ..means anterior band of lateral collateral is gone...thats a grade 1 sprain...so mobilization and strengthening exercises is the treatment...aaaaaaaaaaaaaa 0 - ArchivalUser - 10-27-2007 D. protected wt bearing A. ankle-strengthening exercises can commence after 3-4 weeks of healing. not initial treatment B. long cast would be above knee. no need to immobilize knee. C. short cast seems reasonable, but aircast is more apporpriate D. pt should have protected wt bearing...ie posterior cast, OCL cast, crutches, ace wrap immobilization, aircast, elevation. E. no surgery on this case. no mri obtained. what would you possible operate on? 0 - ArchivalUser - 10-27-2007 well....PWB is a sensible option...but this is a recurrant injury...he had it twice before...he probably needs peroneal strengthening exercises....I will go by amount of pain in real life...u really dont want to give crutches to somebody who is full weight bearing with only mild pain...but yess..I agree...D can be the answer as well 0 - ArchivalUser - 10-27-2007 initial mngmt.......ddd 0 - ArchivalUser - 10-27-2007 Pt has recurrent injury.
