acute pyelonephritis and acute bacterial prostatitis............oflaxacin for 21 days
acute cystitis ..............ofloxacin for 1-3 days
chronic pyeonephritis...........ofloxacin for 3 to 6 mo

chlamydia and gonnococcal together

ceftriaone 250mg im single dose
doxy 100mg bd 10days

if pregnant
then azithromycin instead of doxycycline

if allergic to ceftriaxone then
spectinomycin and doxy

quinolone are not used and these r the latest recommendations from cdc.
