What are ventillator setting in ARDS & COPD - harry206 - ArchivalUser - 07-22-2009

1. RR
2. TV
3. PEEP

Goals:
RR: titrated to pH 7.3 - 7.45. Permissive hypercapnia is acceptable.
TV: 6cc/kg (using ideal body weight)
PEEP: Start at 5 cm H20 and titrate up while titrating down on FiO2 to maintain SaO2 >= 92% and FiO2 < 0.4

The critical parameter though, is to keep plateau pressure < 30 cm H20.

The vent parameter that has the most impact is PEEP, although it may take a day or so for recruitment. I have had SICU patients up to a PEEP of 20.
