patient, with blurred vision, returned from Africa, seeing halos around lights at night, has itching while showering, O/E: the is corneal deposits, and bilateral diminished in visual acuity and retinal edema, lab shows leukopenia, normal CXR
what drug cause the findings with mechanism of action???

a Digoxin
b cephalosporin
c chloroquine
d steroids
e alcohol

He must've have been given Chloroquine in Africa. This is toxic becuase it breaks hemoglobin and separates out heme inside the parasite-ridden cell. this heme breaks into Fe- Protoporphrin IX ( FP ) . This forms a complex with chloroquine FP-chloroquine, which is highly toxic to cells and autodigests it..Nice question.

The parasite breaks down hemobglobin is what I mean,that forms the complex later with CQ Smile

no digoxin is not associated with leukopenia or normal CXR, patient with Heart failure treated with Digoxin will not going to have normal CXR.
