A 35 yr old woman consults an opthalmologist because of double vision and droopy eyelids. She also has complaints of generalized muscle weakness. IV injection of edrophonium dramatically, but only briefly, reverses her symptoms. This patient's probable disease has a pathophysiologic basis that is closest to that of which of the following conditions?

A. Bullous pemphigoid
B. DM typ I
C. Idiopathic Addison's disease
D. Insulin resistance
E. SLE

B.Antibodies against receptors. Its myasthenia and IDDM , autoantibodies against insulin receptors.

A since it is type II reaction

No. DM type I is due to islet cells damage, so no insulin produced. Try again.

autoimmune E.

Answer is: D.

This patient has myathenia gravis, which was suspected based on the woman's clinical presentation and confirmed with the edrophonium test. MG is an autoimmune disease in which autoantibodies directed against the Ach receptor of the muscle side of the NM junction block the ability of the receptor to bind to Ach. Of the diseases listed above, only insulin resistance is produced by a similar mechanism, i.e., antibodies to insulin receptors block the receptor's ability to bind to insulin.

Bullous pemphigoid (choice A) is caused by antibodies directed against the basement membrane of the skin, which damaged the basement membrane and cause blister formation.

Some cases of DM type I and idiopathic Addison disease are characterized by humoral, and probably cell-mediated, reactions against the cells in the endocrine tissue.

SLE (choice E) has circulating and locally generated immune complexes mediating the pathophysiology.

D=NIDDM?

0 - ArchivalUser - 12-23-2005

hi

i think the answer is A ...bullous pemphigoid...since its the same mech

its not type1 dm..since not all cases r autoimmune
not sle bcoz its immune complex dis
def not d bcoz niddm has no such mech involved
