DCIS in 1st trimester-archer query - surgery24 - ArchivalUser - 06-08-2012 A 35 y/o woman comes to you for a mass in her left breast. Her last menstrual period was 8 weeks ago and a pregnancy test returned positive. Further work up revealed ductal carcinoma in situ. She requests if she can keep the pregnancy and also does not ant to lose her breast. The appropriate response at this time: a)She may undergo lumpectomy with radiotherapy but needs to undergo therapeutic abortion b)She can go for lumpectomy with radiation and can keep the baby c)We can postpone therapy untill the pregnancy is complete d)We can just do lumpectomy for now and postpone RT for the pregnancy to complete e)She must undergo radical mastectomy Is DCIS treated the same as carcinoma in pregnancy, because you do modified radical mastectomy for carcimnoma in 1st trimester. Is answer e or d? please clarify 0 - ArchivalUser - 06-08-2012 DCIS rx is either lumpectomy + RT or mastectomy . RT is not done in pregnancy so mastectiomy 0 - ArchivalUser - 06-09-2012 Thanks a lot sadhan. 0 - ArchivalUser - 06-09-2012 pregnancy and lactation is not contraindication for treatment or operation or therapy . Treatment is accoring do stage BB is my ans . 0 - ArchivalUser - 06-11-2012 but radiation is contraindicated in pregnancy 0 - ArchivalUser - 10-09-2012 ee 0 - ArchivalUser - 10-09-2012 DCIS (Ductal carcinoma in situ)whc means its localized no need to do mastectomy ...lumpectomy should work fine and later on RT after completion of pregnancy so D 0 - ArchivalUser - 10-09-2012 D 0 - ArchivalUser - 10-09-2012 If it were second or third trimester, lumpectomy can be done and RT postponed until delivery. This delay in RT does not worsen prognosis But this is first trimester...too much delay if postponed...mastectomy recommended in first trimester. 0 - ArchivalUser - 10-09-2012 I agree docriti with answer D. |
