in a young woman with hx of rheumatoind fever ,there is an early diastolic murmur in left 4th intercostal space near sternum ,better heard in left lateral decubitus position.

what's the murmur?

mitral regurgitation is heard in apex and AR is heard in left 2nd intercostal space .

DIastolic murmurs--->mnemonic "DIe in your ARMS" AR-Aortic Regurgitation MS- mitral stenosis

In rheumatic fever initial heart lesion is a Mitral regurgitation which becomes a Mitral stenosis later (after a few years) so since this woman has a Hx of rheumatic fever..her lesion is a mitral stenosis (diastolic murmur) not Mitral regurgitation ( systolic murmur)

The answer is mitral stenosis .

Systolic murmurs : MR and Tricuspid Reg , aoric stenosis , mitral prolapse , ventricular septal defect

Diastolics : mitral stenosis , aortic reg

Continuos : PDA

The complication of rheum fever is mitral stenosis . I learned the murmurs this way and it helped me a lot .....

@cardio,
the site of the murmur is in 4th intercostal space near sternum,
MS best heard at apex .
please explain...
