Transitional Year gives you the advantage of choosing any specialty after you complete the one year whereas Preliminary Year is like you need to go with only a specific specialty. Prelim Year is commonly associated with 4 year programs like Anesthesia, Radiology etc...

TY is more competitive than prelim because of the wide array of options of which specialty to choose from after the one year. For Prelim, you need to get matched for a prelim year starting in 2007 PLUS also get matched for your choosen specialty like anesthesia or radiology starting in 2008.

TY=Transitional YEAR, not transitional YEARS. So, yes, TY is one year.

Transitional year: A one year of clinical rotation. More than one department you have choice to select. Not that much hard work, not much moonlight. Just like an internship in INDIA.
People do that who is not sure which branch to select in their life.... the will explore the medicine in this one year and find out the way hopefully.

Prelim Year: A one year residency. Specific branch. Just like a resident you have to work but only for a year. Some speciality like Neuro, radio, rad onc want one year prelim IM before their categorical programe to get oriented with medicine. Some hospital allow transitional year instead of prelim IM but most need Prelim IM. So you have more calls , more duties, and more responcibility than transitional year. People do this who knows where he wants to go and people who do not make through categorical prog.

I personally suggest if you r not sure go for transitional year but in you thing you wont make for categorical this year go for prelim IM or the branch you r intrested in.
Good luck
hope it helps
