1 - Is it mandatory to have all the LOR dated within last 6 month ?

2 - Is there any specific dimensions of the paper copy of each of documents to be sent ?

3 - Any specific dimensions of the photograph to be sent ?

4 - Is it safe to send 4 LORs and allot all 4 to every program u apply so in this way u dont miss some who need 4 lors though majority need 3 lors

go to documents ..it has all the answers for your questions.. You can send as may LORs as you need to ECFMG. Then you can choose 3/4 according to program requirement..
