i know of one my colleague presented in ortho...in final year.he had trouble always n ....the patient had fracture rt forearm........patient says..i fell from stairs.....this fellow writes down in his notes...h/o fall.......then patient says difficulty to eat because of the fracture.......this fellow writes sincerely........H/o dysphagia...............GOSH when he presented we all fell on our knees including the proff lolling.....
