I have ECFMG certificate now.
but is it possible to join USA psychiary fellowship(maybe clinical fellowship) and gain the state liscence as a general physician not a psychiatrist?

I think you need to get a Family Medicine residency training, not really psychiatry but doublecheck this with FSMB. I believe you are trying to take the short cut but I am not sure if that is possible. You can try calling up the State Board that gives the medical license.
