dka.......change of fluids question - rollerguy

guys i have this silly doubt.
when glucose reaches 250, we need to change the fluids right.
do u change it d5 ns or d5 1/2 ns????

please reply asap. exam day after!!!!

Hey,
You change it to D5 1/2 NS but make sure you add KCl (10-20mEq) as well!

U wud start KCl from the beginnning if Serum K+ was normal to begin with (usu in DKA, it shd be elevated... a normal value in a DKA patient wud infact imply an underlying hypokalemia). However, if the K+ value is elevated, then there is no need to start KCl from the beginning- u can do so once BSL is 250 or less.

I would type D5 1/2 and KCL in the same order (even if K+ level is normal at present)
