Oh man it trick me also
but why can't it be volvulus
any guaic + in elderly , I will automaticcaly think about Ca but in this case what is the most common cause of large bowel obstruction in elderly : volvulus might be the answer.what do you think?

I think you volvulus-acutely sudden onset and pain is more dramatic. colon ca-induced large bowel obstruction has progressively worsening symptoms-chronic or subacute (this patient-3 weeks time frame) end point of bowel obstruction have similar presentation, regardless of the underlying cause, partial or complete obstruction via imaging studies guide treatment plan.
