Therefore it is not D

Examination shows marked jugular distention.

That means that blood can™t go through right heart

The lungs are clear to auscultation.

That means that left heart is working OK.

Based on these we can exclude A, B, C, F, H

previously healthy + sudden onset of severe shortness of breath

Sounds like Pulm embolism

S4 and a right parasternal heave = overworked right heart + Ph 7.5 PCo2 16mmhg PC2 64mmhg

That™s have got to be Pulm embolism
