Will Warfarin Affect APTT? -

In the first aid, it is said warfarin affects PT only. but since it antagonize vit.K and vit.K is involved in factors II, VII, IX and X, i think warfarin should affect APTT as well.

How do you guys think?

according to goljan, Heparin and Warfarin affect both, but they affect one more than the other, hence clinically there is a "favourite" test to monitor each

It is a good Q though. Classically they used to say so, but it is true and the factor 2 and 10 which are for the common pathway are allso affected, but there is a point, the half life of facotr 7 is just 14 hours but for the formers is much longer, and also production of factor 7 is strongly affected by warfarin, but not that much for 2 and 10, therefor if you assess PT and aPTT of a patient who is under Coumadin, you will see that INR is for example 2 or 3, but the aPTT is just 20% increased.

Many pts after MI, stroke, chronic afib & intermiitt claudication are put on both of these. So u should monitor, INR,PT, PTT.
If they are put on a combination of ASA, heparin, lovenox, coumadin. The side effects are GI bleeding & many others.

Good points. But if it appears in real test, would you consider Warfarin affects PT only?

Haywood, if you had both options choose it otherwise PT is the answer, if time was mentioned which was for a long time or the amount of increased aPTT was also mentioned which was less than 50% choose aPTT also (if applicable).
