a pt w/ typical MI w/ ST elevation in 2 continuous lead, w/o thrombolytic contraindications. what's the best next step magt?
a. tPA and admit to ICU
b. emergency PTCA with stent
uw ans is b.
I think in Dr. Goljan step 2 audio he stress the Tx go for "Cheap", also in Dr.Fisher's highlight video he also says thrombolytics is still the first line, emergency PTCA is a local practice pattern (his hospital angiograph everything), and PTCA should be considered only when thrombolytics are contraindicated. Is it UW's mistake or this is the new recommendation from the American Heart Association?
