Forms 344 and 345 -

Can anyone tell me if i have fill out these forms? I am completing the form online and it asks me if i have filled forms 344 and 345. Do i have to send these forms with form 185.

The application will show u the forms on the same page.U just need to click on then and print them out.You need to send them also along with form 185.It's a must.

when i applied for the step1 exam i didnt fill the above forms . me r u talking about the step2 cs and ck

bj,when did u apply for step 1?I applied in October and they said the 2005 application has these forms.one is a Medical credentials form and another is a medical school release letter.Its not much just simple info about you.They requested that u send it with the Form 183 so I did.I don't know about if u r not a graduate as yet though,I am one. |
