DM II - new onset - hoto or aissman - kidneydoc

Can anyone give me an input how to manage a pt with new onset of DM 2? therapy?

pleaaaaaaaase hoto do not answer this tonight, answer it tomorrow after your test, no offense to you kidneydoc, but hoto needs to relax for the rest of the test tomorrow....
manage him,

fasting

CBC
CMP
Urine for Microalbumin
Good retina examination -- Opthalmologist referral
make sure he is on Aspirin
Hemoglobin AIC,
I like to get a TSH
Metformin
ACEi -- Enalalpril
Dietary consult
fasting Lipid panel

I am sorry if it is too general, its definitely not very well organized, but honestly in any setting, this is all that you really need. Hoto probably knows much better, as he is much more fresh with step 3, but he needs to rest up for tomorrow........

cbc
bmp
finger stick glucose
hba1c
urine for microalbuminuria
lipid profile
opthal consult
and also depending upon age other screening
may be also an baseline ekg..not sure of this

metformin
patient education, diabetes
diabetic diet
diabetic foot care
home glucose monitoring
and other routine counselling

correct me if i am wrong

Genisis..do we need to keep the patient on enalapril ,,i guess thats only when the pt has hypertension or when there are signs of renal failure due to diabetes...

agree with Sandya's management...
