I think dialysis should be the next step....however no harm in doing a ABG to know exact acid base status...may be CCCCC...

This patient with chronic renal failure is manifesting typical signs and symptoms of uremic peripheral neuropathy.Sensorimotor neuropathy in a "stocking and glove " distributuion is the most common form.Usually peripheral neuropathy develops when GFR falls below 10% of normal values.This is one of the indication of starting renal replacement therapy(i,e dialysis).Parenthetically treatment include pericarditis,coagulopathy fluid overload not responsive to diuresis,hyperkalemia resistent to dietry restrictions,severe acidosis and neurologic complication.

Arterial blood gases (which i marked as wel)(choice C) would be appropriate if metabolic acidosis were suspected.
