I am living in NYC. I understand your concern. You have to be very careful not to be late. There's a bus going directly from the airport to the hospital. (Fee is $ 2, make sure you have coins) but it may take so long to get there by bus. A cab would be a good alternative ( should be around $30 )
But the biggest issue is, if you want to travel the same day is the traffic, in NYC the rush hours are always awful so do your best to avoid them.
If IV ends at 4:00 pm should normally be at the airport (JFK, the closest) by 6:00 pm
