its hyperosmolar......so u first give ns .......then replace with .45%nacl...
there is inc glucose>1000 so u have to get it to normal...then there is inc na also seen so u slowly replace fluids instead of abruptly correctly sodium corrct me if i m wrong

so initially we will neutralize the electrolyte then 45p nacl . right . now when we will use potassium and 0.5 % and which pt ? why??

after .45 u look out for glucose ...if it falls replace it with admin of insulin ....with insul k is taken care off....

5% given to prevent cerebral edema, after given insulin when glucose less then 250mg.
potassium is given in all cases no matter its up or down , can couse arrythmia . |
