This patient has a classic presentation for mononucleosis secondary to infection with EBV. The finding on peripheral blood smear is atypical lymphocytes with clumped chromatin and irregular nuclei.


Knobs on RBC membrane and intraerythrocyte organisms is a finding on a peripheral blood smear in a patient with Malaria.

Microspherocytes are a classic finding on a peripheral blood smear in a patient with hereditary spherocytosis.

It is unlikely that this patient would have a completely normal peripheral blood smear.

A patient with Chronic Myelogenous Leukemia would have a peripheral blood smear that demonstrated all stages of granulocyte maturation. It is unlikely that this patient has CML since the mean age of onset is 50 years old and it is slightly more common in men
