Different Patient encounter taught by Kaplan - watano_yo

As from FA and UW, it will start like this,
"Hello Mr./Ms. X, I am Dr. Z. .... How may I help you today?" Then you wait for the SP answers and give s out the CC.

But my friend told me it is not right after taking the Kaplan 5-day CS courses.

The kaplan teacher insists that you never ask SP for CC since it is already on the doorway information.
So they prefer to start like this,
"Hello Mr./Ms. X, I am Dr. Z. ....I notice that, from your chart, you are not feel quiet well and having a chest pain/soar throat etc." The reason why they instruct to do so is that If you are saying somethig like "how may I help you", the SP may responses like "Don't you check my chart? I already told nurse I have a chest pain/soar throat"

I am so confused. What should I follow? Is there any test taker can verify it?

well u could go like this...the nurse told me u have a sore throat...but i would like to hear it from u...wat problem has caused to come in today

or u could say.....the nurse told me u have a sore throat..is that rite ...could u tell me a little more abt it
