No 1) assess wether the patient has hyperthyroidism /not---- first step in mgmt is S.TSH

no 2) a)if TSH is NOrmal ( cold nodule ) has a very high chances of being malignant so ----- next step in mgmt would be FNAC (if palpable) and USG GUIDED FNAC (if not palpable)

b) if TSH is LOW ------- NExt step would be to do a RADIO iODINE UPTAKE SCAN ------if this scan shows nodule is cold-------next step would again be FNAC ,,,,,, if the scan says the nodule is hot----no need for FNAc ?anything

c)if TSH is HIGH --------implies primary hyperthyroidism------NEXT step in mgmt would be SERUM T3 /T4 to confirm and treat wid antithyroid drugs.

And in all if result is MALIGNAT /SUSPICIOUS/FOLLICULR ca ------next step would be surgery
and otherwise jusr follow up wid thyroid hormone levels and observe .....

reshelp08, a correction in what u said...........
point c) if TSH is high, it implies secondary hyperthyroidism....... (central / pituatry cause)
