2) then talk to his boss

3) then licensing board

Ans : D.report him to the state licensing board.

Reports of physician impairement should go first to the local supervisory personnel. If this is either unsuccessful or not applicable then report it to a higher level such as a state licensing board. Telling your own division head is inappropriate because that person supervises u & not potentially the impaired physician.
