can externship be done in B1 /2 visa ststus.
can externship be done in doctors office also or only in teaching institute.
do both hav same weightage

Most colleges dont offer externships,observerships but demand it from the residency program applicants. Most people do wot is called observerships in hospitals and private clinics .They do have some weightage but its mainly d scores that get you IVS
