what document are needed for step 3 and what documents need to be notarized?

only the certification of identification form with your picture, it's the only document you need to send, they have proof of your certificate.

is step 3 exam closed for 1st 2 weeks of Jan or not? Can i schedule for Jan 9th -10th ? thinking of extending the eligibility period........anyone?

You won't be able to schedule your exam for the first two weeks of january, the first available date is january 15.
