Need help on Physical exam and patient note - docm

Please can anyone from this forum tell me how to report the physical exam on the sheet because for example if I have an Abdominal pain as Chief complaint and beside abdominal exam I also perform CV exam , and most of the time I run out while transcribing the PE, should I transcribe for that case only the abdominal exam because it is a focused PE, or I should put everything. Please I welcome any answer.

History and PE both are supposed to be focussed. So if time seems to be a constrain restrict to the system with CC.

if you use abbreviations and write bullet style you'll have time in 10 min. Write everything you asked/did in PE, it's important.
