programs offering observership ask to show immunization proof .........Is there a special format or we can show it on a simple outpatient slip.........Also for some vaccinations like polio ,mmr which I took in childhood I have no documentation , so what to do for them?????In my country OPV is given not the IPV so what to do about it ............kindly help
