A 36 years old male presents to the emergency with h/o multiple stab wounds in chest and abdomen with a 6 inch knife. there is no significant visible bleeding. vitals are BP 60 palpable, PR 120 feeble, temp 98F. upon examining he is pale, cyanotic and cold. JVD is +ve. breath sounds are decreased on the right side. what is the next best step:
A. pericardiocentesis
B. x ray chest expiratory film
C. x ray chest inspiratryfilm
D. large bore needle placement in the pleural space
E. N/S infusion from lage bore IV line

d? i guess its because of the decreased venous return intension pneumothorax that the CO is low..could be E too

the cause of shock here is tension pneumothorax.. i think d should be the right answer

bt uw says always 1st gv 2l iv fluid....if dont correct ...den rx acc to site of bleeding..

dddd..iv fluids are not first step as there is hemothoroax here which is where bleed occuring so that shud be corrected first then iv fluids
