J1 visa by ECFMG - speedyron

Hello,
Can any one tell me ,, how to get the J1 visa sponsered by ECFMG?
I read that you need a letter from program for some training ? What does it mean?
It also says that One needs to be ECFMG ertified.. I am expecting my certification by oct09.. do I need to wait to get certified or I can start the process now?

You can start the process only once you are accepted into any residency program, not before that.
