All applicants are strongly urged to complete the DSF and provide it to their letter writers. In addition, all applicants must complete and sign the AAMC Request for Letter of Recommendation/Cover Sheet and provide it to their letter writers. These documents should be submitted with the LoR. The applicant is responsible for providing these documents to the letter writer. If the applicant has not provided these documents, please ask him or her to do so before submitting the LoR to ERAS Support Services

does that mean that we have to get the letter writer signature on dsf as well.plz help i have moved out of state and have no contact with letter writer now

this is only needed if you want the letter writer to send the LOR directly to ECFMG. in case you are sending the LOR to ECFMG these are not needed EXCEPT the DSF.

if you are sending directly then send DSF Along with the LOR (of course) to ECFMG for scanning.
