you start which of the following first:

1. regular insulin
2. NPH

no regular (=short acting) for DM 2
---------------------------------

usually,

twice intermediate (NPH/lente)
or,
once long acting (glargine/lantus)
---------and,

meal time ultrashort (lispro/aspart) if require.
