I ve been approved a 5 years multiple entries B1/B2 visa but in my ds form i wrote that i ll stay for 1 week just to give the STEP2 CS exam and go back to my country.

But now, i would like to do an observership to practice my english and get used to the US system and i need to stay more than a week. Can i do ,guys?? I ask just to avoid some kind of issue by the time i apply for J1 or H1.

yes you can. Much depends on how long you are granted to stay at the port of entry, which usually is 6 months. You can show the officer at port of entry a copy of your observership letter or letters of interview calls. Just convince them that you need to stay that long..
