I would like to know what to do if in the past you tried to apply for the exam and for whatever reason(Lastname on the application did not match the one in the medical credential because did not skip a space when filling out the application) you never got a block or date, but you had alreday pay when it was $615 to take the exam. Should I apply and send the difference since I heard that the price went up. I know I need to provide them with more documentation with regards to the lastname thing. Please help. Thanks

yes, apply again with the correct information this time.
they kept that money in your account, you didn't lose it. but yes, you may have to pay the difference. you can call them and find out about it.
