ECFMG certification is not useful for anything but residency. You can't work as a physician without step 3 and without residency training. There are free clinics that you can volunteer in but you wont get paid and you will not be allowed to do any doctoring.

For the BOP idea it is a little more complicated. This is from the BOP website
http://www.bop.gov/jobs/job_descriptions...fficer.jsp
"Basic qualification requirements for all Medical Officer GS-11 positions: Doctor of Medicine (M.D.) or a Doctor of Osteopathy (D.O.) from a school in the United States or Canada approved by a recognized accrediting body in the year of graduation, or permanent certification by the Education Commission for Foreign Medical Graduates or a fifth pathway certificate; and a one-year post M.D. or D.O. supervised internship or the first year of a residency training in an accredited institution; and possession of a permanent, full, and unrestricted license to practice medicine in a state, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. If not currently licensed, you must obtain a license before entering duty; you may be selected for appointment pending meeting the licensure requirement; however, you must obtain a license before entering on duty."

Cool. This could be an option for people working in dead end prelim positions.

within the usa, you cannot utilize ecfmg certification for anything other then application, and perhaps gives you some recognition that you have passed professional exam. by itself it is not a degree, it is of the undergraduate level and doesn't reflect specialist level.
