Some programs with a 5 years cut off are not strict about them , for example this program in NY stated in their website that ( Do you have a Medical School Graduation Date cut off?
Preference is given to those that have graduated within five years of the anticipated training start date, however the committee will take into consideration what the applicant has been doing since graduation from Medical School if more than 5 years)

Of course there are a lot of programs that don't have a cut off or some of them have a cut off but more than 5 years. So can you please provide us with a list for these programs
