In very crude terms, externship is done away from the school/college of medical education. internship is hands on clinical training AT your institution.

thank you mleresidencytips, so in my undrestanding, we can do externship in any other institutions other than educational institutions, am I right? what we do in externship?is it hans-on training?

externship has to done at another medical institution. it has to be approved as an away rotation by your medical school and yes it will be hands on

so as a foreign medical graduate I can not take an externship, am I right? is observership the only thing I can do as a fmg?

i don't think there are restrictions on fmg/img doing externships.
you will need the necessary documentation: insurance, appropriate visa stuff etc

there are no restrictions on getting externship for IMG. I remember, back in medical school i had a friend who came to baylor to do externship in medicine for 12 weeks. He had to arrange for a lot of paper work though
